Accidents
Most accidents that occur in the workplace should be investigated as firstly it will reduce the risk of re-occurence and secondly it will avoid problems at a later date if what at first seemed to be a minor injury turns out to be a long term condition. The main purpose of an investigation is to ascertain the facts so that preventative measures and general reccomendations can be made.
Accidents
There must always be a procedure in place for the reporting of accidents, the form of which will largely depend on the size of the company, but which will usually be some form of accident book so that a written record may be maintained.
If an accident does occur then the biggest priority is to come to the effective aid of the injured party. All first aiders should be given support in giving assistance and any emergency services needed should be called atraight away. The patient should not be moved but should be made as comfortable as possible. Once the pateient has been removed the accident site should be isolated with nothing disturbed and an investigation should start straight away.
Accident investigation should follow a set procedure and ideally the section supervisor should carry out an immediate examination. In some cases an external expert will need to be called in (eg. a safety adviser or an engineer). An investigation should be concerned with establishing the facts of the case not apportioning blame. All witnesses should be interviewed as soon after the incident as possible and a distinction should be made between actual fact and hearsay or opinion.
Accident reports
Typical information needed in an accident report:
Name of owner or occupier
Department or section
Type of industry or process
Exact place of accident possibly including a sketch map or photograph if the accident was serious.
Name of injured person, occupation, age, marital status, home address, length of employment with the company, registered disabled, names of witnesses, address' of witnesses, name and address of supervisor in charge of the job.
What was the injured party doing at the time of the accident ? Was the job being carried out authorised ?
Full nature of the injury, where treated, own doctors name or name of hospital.
Date and time of accident.
Probable cause of accident
Outline any preventative measure take.
